NOTICE OF PRIVACY PRACTICES

Healthy Measures, LLC, is required by law to maintain the privacy of Protected Health Information ("PHI") and to provide you with notice of our legal duties and privacy practices with respect to PHI. This Notice of Privacy Practices ("Notice") describes how we may use and disclose PHI. The Notice also describes your rights with respect to your PHI. We are required to provide this notice to you by the Health Insurance Portability and Accountability Act ("HIPAA").

Healthy Measures is required to follow the terms of this Notice. We will not use or disclose your PHI without your written authorization, except as described or otherwise permitted by this Notice. We reserve the right to change our practices and this Notice and to make the new Notice effective for all PHI we maintain. Upon request, we will provide any revised Notice to you.

INFORMATION COLLECTED

In order to participate in a Healthy Measures Screening Program, you must agree to complete a Healthy Measures Screening Form as well as your signature of authorization and consent. After the screening your information may also be used by Healthy Measure s for any of the following purposes:

  • Conducting market research to improve the programs, products, and services offered by Healthy Measures.
  • Conducting routine internal business management operations
  • To verbally provide you (at the screening event only) with information about common health issues, such as high cholesterol, glucose, blood pressure and other possible conditions.

SHARING INFORMATION

Unless required by law, Healthy Measures will not sell or disclose your personally identifiable health information to any third party without your permission. All de-identified information (information stripped of all data elements that could be reasonably used to identify you personally) may be stored and used by Healthy Measures for normal internal business management related purposes. It will not be provided to any other entity unless the entity requesting the information is legally entitled to receive it.

VOLUNTARY PARTICIPATION AND WITHDRAWAL

You can choose not to allow Healthy Measures to collect your information, which may make you ineligible to participate in a screening event. If you give your permission and later change your mind and withdraw your consent, Healthy Measures will block any further release of any personally identifiable health information about you. However, after blocking your name and other personal identifiers, Healthy Measures, its successors in interest, may keep and use any information that you may have provided previously.

This information will only be used for normal internal business management operations. Unless you contact Healthy Measures to withdraw your consent, your information will be stored and used internally by Healthy Measures indefinitely.

OPENNESS

This Notice is part of Healthy Measures commitment to protect the privacy of participants in screening programs. We display a version number and date on the notice so it is easier for you to know when there has been a change. This Notice may be amended as we add programs and services and as laws change.

SECURITY

Maintaining a secure system is in the best interests of our customers, program participants and partners. That is why Healthy Measures stores personally identifiable data using industry standard physical, technical, and administrative safeguards to secure data against foreseeable risks, such as unauthorized use, access, disclosure, destruction, and modification.

Healthy Measures has adopted a code of conduct for employees that are permitted to access our customers’ or program participants’ personal information. Only authorized Healthy Measures employees are permitted to access personal information. All Healthy Measures employees must abide by our Privacy Policy and those who violate our Privacy Policy are subject to disciplinary action, up to and including termination. After a screening event, all information under Healthy Measures control will be stored in locked files and on Healthy Measures computer servers, and only Healthy Measures, its successors in interest, will have access to the information.
