Healthy Measures provides targeted and customized community-based programs to a variety of Medicaid/Medicare Insurance clients for their general and hard to screen, non-compliant populations. We provide testing, results, and education at the point of care to close care gaps and help members achieve improved health and well-being. Healthy Measures provider bills testing for HEDIS measures and customized data reporting so Care Management teams can provide member follow-up.
